Please note:
A credit card is required on file at the time of scheduling. In the event you need to reschedule, please do so within 48 hours or you will be charged a $50 no show/ late cancellation fee. You will receive a superbill at the end of your visit to submit to your insurance company for any potential reimbursement.
Dr. Kapoor will recommend personalized testing during your visit. All testing is optional. The cost of the testing and supplements is not included in the appointment cost.
Dr. Kapoor can see you via telehealth for follow-up visits. Telehealth appointments are available for reimbursement now as well. Please check with your insurance carrier.
Scheduled visits are required for Dr. Kapoor to continue to give medical advice. We request follow-ups be made every 3 to 6 months depending on the recommendations of Dr. Kapoor. If you are getting prescription medications filled by Dr. Kapoor, a visit every 3 months is required.
