Follow Up Visits:
1st Report of Findings Visit: Includes Lab Review and Personalized Treatment Plan with
Diagnosis Specific Protocols.

Extended Report of Findings Visit
75 minutes | $750
Recommended when reviewing labs and one specialty test
90 minutes | $900
Recommended when reviewing more than 2 specialty tests and labs
Subsequent Follow-Up Visits

Comprehensive Nutrigenomics Visit

Please note:
A credit card is required on file at the time of scheduling. If you need to reschedule or cancel, please do so within 72 hours or you will be charged 50% of your scheduled visit type fee for the cancellation. You will receive a superbill at the end of your visit to submit to your insurance company for any potential reimbursement.
Dr. Kapoor will recommend personalized testing during your visit. All testing is optional. The cost of the testing and supplements is not included in the appointment cost.
Dr. Kapoor can see you via telehealth for follow-up visits. Telehealth appointments are available for reimbursement now as well. Please check with your insurance carrier.
Scheduled visits are required so that Dr. Kapoor can continue to give medical advice. We request follow-ups be made every 3 to 6 months depending on the recommendations of Dr. Kapoor. If you are getting prescription medications filled by Dr. Kapoor, a visit every 3 months is required.
